Camper Release Form
Fill out this form so our staff knows when your child will be leaving and who will be authorized to pick them up.
Health History and Examination Form
This form requires a physical within the past 24 months. A school physical or a doctor's signature must accompany this form.
Confidential Information Form
This form will help our directors and your camper's counselor get to know your camper before his/her arrival at camp.
Liability Release Form
A parent or legal guardian must sign one of these forms for each camper attending.
2008 Registration Form
This form must be filled out for each camper attending and submitted with your deposit or full payment.
